Immunizations and physicals offered

There are several exams, tests, and immunizations that your child might need for entry into school this fall. Certain vaccines are required for kindergarten, sixth grade, and ninth grade (if not received in sixth).

The Grundy County Health Department has immunization clinics on Wednesdays by appointment. There are certain eligibility requirements to attend these clinics, which are open only to Grundy County residents, including enrolled in Medicaid, does not have health insurance, American Indian or Alaskan Native, or has health insurance that does not pay for vaccines.

To schedule an appointment please call the Grundy County Health Department Nursing Secretary at (815) 941-3419.  The GCHD asks for a $20 donation per immunization, but no one is turned away if unable to pay.
